Endowment Fund Form

This form sets out your intention to set up an Endowment Fund with Acorn and outlines your wishes for the distribution of your fund. 

This form can be completed alongside your Will and lodged with your solicitor if you are choosing to donate part of your estate after you have died - note that you can make changes to this Endowment Fund Form at any time without needing to alter your Will.

Or you can complete and return the form to the Acorn Foundation if you would like to set up a fund and donate while you are living.  This option allows you to see your 'fund in action'.
